Welcome Packet

Dear Parents,

 

Please find the contents of our Welcome Packet for the start of the 2020-2021 school year.  All of the documents can be downloaded and most are writeable PDFs for your convenience.  Included in this packet are several District required forms, which require your immediate attention.  

 

Please plan to email or upload the following forms by Friday, August 21st:

 

 

Please complete and print a copy of the Responsible Usage Policy form.  Present this form to our staff, if you are coming to campus to check-out a student device.  Device pick up times will be scheduled by grade level and classroom or please contact the Main Office to arrange a time convenient for you.  

 

To submit documents through our website, please select the SUBMIT STUDENT DOCUMENTS FORM under the "Forms" tab located beneath the header on our website.  You will be asked to provide detailed information and can attach your completed documents to your form.  Additionally, if you have multiple students, you will only need to complete this form once.  You may indicate multiple classrooms in your forms submission.
